A friend in the states needs some pretty major dental surgery and is thinking coming over to do so, thinking BKK.

If I were in the states and needed major dental surgery I'd do it there. By the time you buy a round trip ticket, accommodation, dental fees and whatever else is involved it will work out about the same price as in the states. The advantage there is that you can understand what the dentist is telling you and the dentist can understand you. The best Thai dentists study in the states. The best dental schools are in the states. For most dental work Thai's are fine but for major surgery I wouldn't trust them. maybe others have a different view and experience.
The whole medical tourism industry is in decline--greed, particularly by International hospitals, is the reason. The Arabs from the gulf are still an important element but they come for other reasons too.

I don't disagree. He's got the money, and the time, so It's a combo vacation/medical

There are many dentists in Bangkok. Some at the major hospitals, most of them private clinics.
I use Bangkok Smile, but not the main clinic. The one I use is in Soi Sukhumvit 63 (Ekamai). It is in a BigC shopping centre so there is easy parking. A female doctor that speaks fluent English. They can do most procedures there, but can send you to the main clinic if required. Never had problem with her over about 10 years.
Right at a BTS station. The owner is well qualified, but as with many places you take your chances with just which Doctor you get. They have a small parking area, but it is difficult to get into. Main reason why I gave up on them.

Asavanant in Bangkok is one of the best private clinics, but if your friend actually knows what treatments they need then they can look online for specialists in those areas and find what hospitals they work at.
Most of the dentists at the bumrungrad or other big name hospitals also work at cheaper government hospitals sometimes..
