For those who need governmental proof of vaccination to travel to certain countries, Hua Hin Hospital is issuing a Certificate of Vaccination booklet for people who have been vaccinated at their facility.

Speak to the hospital staff and tell them what you want and they will contact the proper department to help in the process. Bring your paper showing both doses are done, your passport and 50 baht for the fee. Wait one week for the hospital to call you to pick up the book.
If you received a vaccination from another facility you should contact them directly for their process to get the booklet.
If you have any questions, please go to Hua Hin Hospital personally and ask.

All those who received a first dose of Sinovac have been promised a 2nd dose of AZ , 3-4 weeks after the first jab. However, after reading your post, I contacted them yesterday asking if I can reverse my decision. I got a reply this morning saying I'm back on the list and will be contacted with an appointment date.
